Insurance Info

Our physicians are participating physicians with Medicare.

We also participate in most insurance plans, such as Blue Cross/Blue Shield, Aetna, United Health Care, Florida Health Care, 1st Health and many others.

Some HMO plans require authorization. If your plan does require authorization, be sure to contact your primary care physician to have that authorization sent to us prior to your appointment. We must have your referral at the time of your appointment.

You may be responsible for any balances, such as deductibles, copays, and any non-covered services (such as Refraction).

As a courtesy to our patients, we will also submit claims to your supplemental insurance carrier. Any balances not paid by these insurances will be the responsibility of the patient.
